See the Form W-4 instructions in this publication for more information. Please note that in some cases, we could require an additional Form 8300, and there will be a separate Form W-4 you must file. See Pub. 1481 for information about Form 8300. If you are unable to obtain a copy of the Form 8300 from the employee, you can get your copy in person from any IRS office or, if you prefer, from the IRS at the address in box 7, line 21.
